Assumption of risk.I understand that participation in Coach K. Ross's programs (Skills & Drills Baseball Clinic, ASD Space Coast Athletes, and Faith International Academy Brevard) involves inherent risks including, but not limited to, being struck by a ball or bat, collisions with other participants, falls, weather conditions, equipment-related injuries, and other unforeseen risks that may result in injury, illness, or in rare cases serious harm. I voluntarily and knowingly assume all risks associated with my child's participation in practices, events, games, travel, and related activities.

Release of liability and hold harmless.In consideration of my child being allowed to participate, I hereby release, waive, discharge, and hold harmless Coach K. Ross, Skills & Drills Baseball Clinic, ASD Space Coast Athletes, Faith International Academy Brevard, and their respective coaches, staff, volunteers, sponsors, and affiliated facilities from any and all claims, liabilities, demands, damages, costs, or causes of action arising out of or related to any injury, illness, loss, or damage sustained by my child during participation. This release applies to the fullest extent permitted by law.

Medical authorization and financial responsibility.I certify that my child is physically able to participate in the activities of the selected program. I authorize Coach K. Ross and program staff to obtain emergency medical treatment for my child by any licensed physician, hospital, or clinic in the event of injury or illness if I am unavailable. I understand and agree that I am financially responsible for any medical treatment provided to my child.

Photo and Media Release

I grant permission for Coach K. Ross and his programs to use photographs and/or video of my child for promotional purposes, including social media, website, and printed materials.
